Continence Service
- stress urinary incontinence - leaking urine when you laugh, cough, sneeze or with exertion, for example, walking or running
- urgency - the feeling of urgently wanting to go to the toilet that is difficult to put off
- urge urinary incontinence - urgently wanting to go to the toilet and then leaking urine before you get there
- frequency - needing to go to the toilet more than six to eight times over a 24 hour period
- overactive bladder (OAB) syndrome - urgency and frequency, with or without incontinence (urine leakage)
- recurrent urine infections appliances - we can assist with appliances/fitting of appliances
- prolapse - this is the dropping down of one of your pelvic organs causing a feeling of heaviness or a bulge from the vagina or back passage
- faecal urgency - problem of needing to open your bowels without much warning
- constipation - hard stools, infrequent opening of your bowels, the feeling of straining, and sometimes feeling like you can’t completely empty your bowels
- faecal incontinence - this is an inability to control wind or stool resulting in leakage - this can also be associated with urgency
- problems following a prostatectomy - after having this procedure the pelvic floor musculature and nerve supply can affect urinary leakage, anal incontinence, erectile dysfunction.

Exclusion criteria: Patients with sudden onset frequency / urgency or urinary tract infection, haematuria (blood in urine) or urinary retention, recently altered bowel habit or faecal impaction, prolapse beyond the introitus or abnormal bleeding. Patients with Pelvic Girdle Pain / Symphysis Pubis Dysfunction (SPD) /diastasis recti should be signposted to MSK physiotherapy.
Patients who require an updated continence assessment or who require advice/guidance regarding obtaining continence products should contact their local District nursing team or GP.

Dementia Support - MCH
- offer short term support and home treatment for challenging behaviour and physical problems such as infection or after a fall where emergency services are not required
- assess a patient's current physical and mental health needs
- support carers and enhance their skills
- support early discharge of patients admitted to Medway Maritime Hospital or KMPT by managing the patient at home and supporting the carer
- signpost to other services when necessary
- provide training and support in managing dementia and acute exacerbation of the condition to community nursing teams and care homes
Referral criteria
The dementia support team will be available to adults with a diagnosis of dementia or who are in the process of a diagnosis being made.
Patients must also be registered with a Medway GP.
Referrals can be made by a GP, health professional or carer.

Dementia Support – Admiral Nursing DIRECT
Admiral Nursing DIRECT is a national helpline and email service provided by Dementia UK which is for family and professional carers, people with dementia and those worried about their memory.
Admiral Nurse service in Medway is provided by Medway Community Healthcare in partnership with Dementia UK.
Admiral Nurses are specialist dementia nurses who give expert practical, clinical and emotional support to families living with dementia to help them cope.

Dental - emergencies
Out of hours emergency dental service is known as DentaLine and is for dental emergencies and urgent symptoms during evenings, weekends or bank holidays. To contact the service, you should call on 01634 890300 for an assessment, advice and to arrange an appointment for that session if necessary. Patients should contact their own dentist for dental emergencies during practice opening hours.
Our call centre is open: Monday to Sunday from 6.00am to 10.55pm and Saturday, Sunday and Bank Holidays from 8.30am to 1.25pm
If you meet our acceptance criteria an appointment may be offered, but if daytime care is felt to be more appropriate, you will be advised. Treatment provided by DentaLine prevents deterioration in a patient's oral health or will give temporary control of pain. After treatment, patients are still required to return to their own dentist, or join the list of a dental practice, for a new course of treatment to permanently resolve their dental need. Our service accommodates patients who have the following dental symptoms:
- haemorrhaging (bleeding from the mouth);
- trauma - Following a recent Accident or injury;
- severe facial swelling (affecting the airway or eye);
- sudden onset of acute pain not relieved by over-the counter pain killers.

Healthy Living for people with type 2 diabetes: direct GP practice and self-referral routes now available
People living with type 2 diabetes and their carers can now self-refer via the Healthy Living NHS website. GP practices can continue to refer patients via a referral form, which is available on Ardens systems under diabetes templates, other GP systems may need to upload it manually.
Healthy Living is a web-based structured education programme that provides self-management information to people living with type 2 diabetes and supports improved health and wellbeing.

The helpline is a telephone service available to adults (aged 18 and over) in Kent and Medway who are concerned about their mental health, or the mental health of a friend or relative, and need urgent help.
If you are working with someone who you believe urgently requires specialist mental health care or treatment from KMPT you can refer them to the Urgent Mental Health Helpline for a triage call. If it is not urgent (needing a response within 72 hours), routine referrals should be made by the individual’s GP to the appropriate community service.
